Personal Care
Services

Learn about PCS

The PCS program is designed to provide personal care services to individuals that need assistance in their effort to perform their activities of daily living that include bathing, dressing, mobility, toileting and eating.

Community
Alternatives Program
for Disabled Adults

Learn about CAP/DA

CAP/DA services allow the beneficiary to remain in or return to a home- and community-based setting by supplementing, rather than replacing, the formal and informal services and supports already available to an approved Medicaid beneficiary.

Community Alternatives
Program for Children

Learn about CAP/C

CAP/C provides a cost-effective alternative to institutionalization for Medicaid beneficiaries who are medically fragile and at risk for institutionalization if the home and community-based services approved in the CAP/C waiver were not available.

Pre-Admission
Screening and Resident
Review Process

Learn about PASRR

Pre-Admission Screening and Resident Review (PASRR) is a federally required screening of any individual who applies to or resides in a Medicaid-certified nursing facility, regardless of the source of payment.

Transition of Care
Coordination

Learn about TCC

The Transition of Care Coordination Program is responsible for providing community options for transitioning beneficiaries from institutional care to community-based living and for beneficiaries transitioning from managed care to Medicaid Direct.

Acentra Health is proudly partnered with the North Carolina Department of Health and Human Services, Division of Health
Benefits to administer the NCLIFTSS (Linking Individuals and Families to Long Term Services and Supports) program.

Acentra Health will help enhance quality through increased provider training and technical support, outreach to under-
served LTSS eligible beneficiaries, and coordination between LTSS and benefit plans.
