Pre-Admission Screening and Resident Review
Pre-Admission Screening and Resident Review (PASRR) is a federally required screening of any individual who applies
to or resides in a Medicaid-certified nursing facility, regardless of the source of payment.
This requirement was enacted to ensure individuals with serious mental illness (SMI), intellectual or developmental
disabilities (I/DD) and/or related conditions (RC) receive appropriate placement and services.
A Level I identification screening must be performed before anyone can be admitted to any Medicaid-certified nursing facility.
When the Level I screening indicates the possibility of SMI, I/DD or RC, a Level II, in-depth evaluation must be performed to assess for nursing facility placement and for potential specialized care needs of the individual.
PASRR screenings must also be provided for previously admitted individuals who have demonstrated a SMI, I/DD and/or RC significant change in condition.
NC Must
- NC Medicaid uses an internet-based screening tool to manage the PASRR Program. The North Carolina Medicaid Uniform Screening Tool (NCMUST) user interface allows referring and admitting agencies to communicate and manage PASRR screens in a self-service application. NCMUST uses an automated decision service to establish the appropriate PASRR level. The assigned PASRR authorization is usually generated in seconds and is viewable in NCMUST. If manual review of the screen is required, the screen is routed to a queue monitored by NC Medicaid PASRR nurse consultants. The NC Medicaid PASRR nurse consultants can then communicate back to the originator of the screen (screener) as needed to make a clinical decision.

Resident and Applicant Information
PASRR is a two-part screening process. There is a Level I screening which may indicate that a Level II assessment is needed. The Level I screening is looking for evidence of a serious mental illness, intellectual disability, or a condition related to intellectual disability. The purpose of a Level I screening is to identify anyone who may need a PASRR Level II evaluation.
The purpose of a Level II evaluation is to confirm that an individual has a serious mental illness, intellectual disability, or a related condition and to assess the need for additional services. These services are used to support people while they are also receiving nursing facility services.
Acentra Health works with the State of North Carolina to complete the PASRR Level II evaluations. These assessments will be in-person, at the location of the person being screened and will be conducted by a qualified mental health and/or intellectual disability professional.

Our team of PASRR Evaluators utilizes a multi-disciplinary approach and consists of highly skilled and trained professionals, licensed in the state of North Carolina, including master's level psychologists, nurses, social workers, and other mental health professionals. We work together to identify individuals with possible serious mental illness (SMI), intellectual disability (ID), and other related conditions (RC) and provide comprehensive evaluation prior to admission to nursing facilities. PASRR Level II evaluations assess nursing facility level of service needs and additional care needs, such as Specialized Services and Specialized Rehabilitative Services, to ensure individuals with SMI, ID, and RC receive needed services in the least restrictive setting possible.
Acentra Health adheres to the Federal and State regulations governing the PASRR process. The Level II evaluations are sensitive to each individual’s culture, language, ethnic origin, and means of communication. Our determinations are person-centered and support individuals in need of additional services while in nursing homes.
